Financial Policy

Patients Responsibility

 

You are financially responsible for the services we provide you.  We expect co-payments, co-insurance and deductibles for office visits at the time services are rendered.  Anesthesia charges performed by an anesthesiologist may not be reimbursed by your insurance carrier and you may be responsible for their bill.

 

If your insurance company requires a referral for your office visit, you are responsible for obtaining this referral.  If you do have the proper referral you may be required to reschedule your appointment.  We accept cash, check, Visa and Mastercard.  We do not accept American Express or Discover.

 

Returned Checks

 

There will be a $25.00 charge for any and all returned checks by your bank for any reason.

 

Missed Appointments and No Shows

 

We see patients on an appointment basis.  We request that you call in advance if you must cancel a scheduled appointment.  Unless extenuating circumstances prevail, you may be assessed a $25.00 fee if you fail to give 24 hour notice of a cancelled appointment.

 

 

Information Change

 

Please advise us of any change in your insurance coverage, home address or telephone number immediately.
